The Consultation Process

Thank you for considering The Effra Clinic for an assessment. In order to ensure that we are in a position to help you please read the FAQ as they provide important information about a range of questions you may have about the process, the fees and nature of the appointments. 

The first step to contact us is by completing and submitting the short enquiry form so we can send you an encrypted registration form. By completing this you will provide the information we need for the triage of the inquiry and we will let you know if we are the appropriate service for you. 

 

Following acceptance of your inquiry by the triage clinician, you or your child will be placed on our waiting list. We will let you know in writing about every step of the process. An appointment will be offered to you as soon as we can but please keep in mind that the demand is at times very increased and because we are a small service, sometimes the waiting is more than we want. We will keep you informed if there are unexpected delays.

Please keep in mind that if you are a parent of a young person between 16 and 18 years old inquiring on their behalf, we will need their explicit consent before organising any assessments/ appointments. Please complete the enquiry form and our admin team will guide you accordingly. 

Please note that the Effra Clinic is not an emergency/ crisis service. Therefore, we cannot respond to crisis situations or offer urgent appointments. In those circumstances you must seek help from your GP or local A&E services.

Please also note that we cannot accept referrals from anyone other than the person who would like to be assessed (eg, not parents, partners or assistants) and they will need to enquire with us directly.”

Repeat 
Prescription
Form

For patients who receive follow up in clinic, please use this form if you need a repeat prescription prior to your next appointment. Prescriptions can either be sent to your home address or to a nominated pharmacy via special delivery. Please allow 5 working days for prescriptions to be issued.

 

There is a £50 fee for controlled drug prescriptions issued outside of clinic appointments and a £30 fee for non controlled drug prescriptions. You will be invoiced once your prescription has been posted.  

 

Please note that this is for repeat prescriptions of medication regimens agreed in clinic, if you need a change of medication or dose, this will need to discussed with your clinician in clinic. If there are any questions about your request from the clinical team we will be in touch via email.

Privacy Policy

This form collects your name, date of birth, email, other personal information and medical details. This is to confirm you are registered with the Effra Clinic and to allow the practice team to contact you and access your correct patient record. Please read our Privacy Policy to discover how we protect and manage your submitted data.

Please note: For reasons of privacy this form will not store your details or medication request. There is no email acknowledgement with this service. Once you send this form a notification message will appear to indicate successful submission.
